B型肝炎ワクチン出生時接種、陰性母親の乳児では個別の共同意思決定を推奨――米国ACIP

米国疾病予防管理センター（CDC）の予防接種諮問委員会（ACIP）は、B型肝炎ウイルス検査が陰性の母親から出生した乳児へのB型肝炎ワクチン接種について、個々の状況に応じて医療者と保護者が話し合って決める「共同意思決定」を推奨することを採択した。

保護者と医療者は、ワクチンの有効性や副反応のリスクに加え、B型肝炎患者が同居しているか、高い有病率地域からの出身者と頻繁に接触するかなど、感染機会も考慮する。

出生直後の初回投与を行わない乳児では、最初の接種は生後2か月以降とする。

ACIPに示された資料では、新生児の主なリスクはB型肝炎表面抗原陽性の母親からの感染であり、過去の急性B型肝炎減少には普遍的な出生時接種の寄与は比較的小さいと報告された。
