都内のとある大学で、有志のみなさんが定例開催されている勉強会に読んでいただき、講演させていただきました。
半年前からご依頼を受けていたもの。
「命と政治ーなぜ医師が社会を診なければならないのか」という大変重いテーマをいただき、何をお話しするか迷いに迷ったのですが、私が医学生時代から、そして医師として医療の課題に向き合ってきた35年程の歩みを振り返り、どんな現実に触れどんな問題意識をもち、怒り葛藤し悩みながら行動してきたかについて、率直にお話しすることにしました。
こじんまりした会だから気楽に…と言われていたのですが、伺ってみると医学生や研修医、院生、看護師さんとともに、とある科の教授や研究者の方、困窮者支援の現場で活動されている方など多彩な方々に多数ご参加いただき、熱心に質問も出していだき、私自身が多くの学びを得る機会となりました。
特に、若い方々から次々に繰り出される真剣で本質的で鋭い質問には、さすが…と舌を巻きました。
#健康の社会的決定要因 ということが言われるようになって久しいですし、Solid Facts(確かな事実)として、貧困と格差をはじめ健康に及ぼす決定的要素が、少なからず政治によってもたらされ規定されていることは、医学・医療の世界でも疑いない事実として認識されてはいるのですが、なかなか普段の生活の中で…診療の中で…政治を意識し関わる機会は少ないからでしょうか。私のような「政治」に向き合い関わり続けてきた、ちょっと変わった医者に関心を持っていただいたのかもしれません。炊き出しの現場などでお会いできる方がうまれたら嬉しいなと思います。

贫富差距并不是制度错误!而是三维世界的底层设定。在这个以物质为基准的维度里,资源有限、能力不均、机会不等……差异叠加后阶层结构自然形成。制度可以调节差距,却无法让差距消失。革命可以重新洗牌,却无法阻止世界再次分层。

The wealth gap is not a systemic flaw! It's a fundamental setting of the three-dimensional world. In this material-based dimension, limited resources, unequal abilities, and unequal opportunities... these differences, when compounded, naturally create a class structure. Systems can adjust for these gaps, but they cannot eliminate them. Revolutions can reshuffle the deck, but they cannot prevent the world from becoming stratified again.
The price of all factors of production stems from their marginal contribution to satisfying consumer demand (the consumer's subjective perception of value, since value itself is subjective; there is no objective value, as exemplified by the value of water and diamonds), and is ultimately determined by entrepreneurs' expectations and bidding.
