ワイングラスを全て空けた長女は、軽く服を着替えながら優雅に執事の案内を受け、客を迎える準備のために準備場所へ足を運んだ。その後を影のように執事が上手に足音を合わせ、彼女が飲み終えたグラスを自然に手に取り、客名簿を持って彼女に付き従った。
「今日はみんなを満足させる夜になることを願うだけだよ…」
と独り言を呟きながら、彼女は悠然と歩みを進めた。屋敷の天井に張られたガラスに反射する月明かりが、彼女の孤高な姿を静かに照らし出し、蜘蛛の巣が織りなす幻想的な影がその輪郭をさらに際立たせていた。準備部屋に到着すると、執事は客名簿を手に持ったまま、静かにドアを閉めた。長女は鏡の前に立ち、新たなドレスに袖を通しながら、ふと窓の外を眺めた。遠くで馬車の音が聞こえ始め、客たちの到着が近づいている気配がした。
「楽しみだね…でも、期待が大きすぎるのも厄介かも…」
彼女の声は低く、どこか自嘲気味に響いた。執事は一礼し、
「お嬢様、すべてお任せください」
と静かに応じた。その瞬間、廊下からかすかな足音が近づき、ドアがそっとノックされた。長女は微笑みを浮かべ、執事に向かって軽く顎を振った。夜の幕が上がり、彼女の瞳に宿る光は、満足させるだけでなく、何かを試すような鋭さを含んでいた。月明かりが再び彼女を照らし、屋敷全体が静かな期待に包まれた。

Having emptied all the wine glasses, the eldest daughter gracefully accepted the butler's guidance, lightly changing her attire as she headed to the preparation room to ready herself for the guests. The butler followed like a shadow, skillfully matching her steps, naturally taking the empty glasses before trailing her with the guest list in hand.
“I can only hope this will be a night that satisfies everyone…”
she murmured to herself, walking with unhurried elegance. The moonlight reflecting off the glass ceiling was the only thing illuminating her solitary figure, the spiderwebs casting a fantastical shadow that accentuated her silhouette.Upon arriving at the preparation room, the butler quietly closed the door while holding the guest list. Standing before the mirror, the young lady slipped into a new dress, her gaze drifting to the window. The faint sound of a carriage approached from afar, signaling the nearing arrival of the guests.
“I’m looking forward to it… though too much expectation might be troublesome…”
Her voice was low, tinged with a hint of self-mockery. The butler bowed and replied softly,
“My lady, leave everything to me.”
At that moment, a faint sound of footsteps echoed from the corridor, followed by a gentle knock on the door. The young lady smiled and gave a slight nod to the butler. As the night’s curtain rose, the light in her eyes carried not just the intent to satisfy, but a sharp edge as if testing something. The moonlight illuminated her once more, enveloping the entire mansion in a quiet air of anticipation.

【「患者さんと話せている＝SDMができている」ではありません】

SDM（共有意思決定）は「臨床医と患者が、意思決定という課題に直面した際に最良の科学的根拠（エビデンス）を共有し、患者が選択肢を検討し、十分な情報を得た上での選好に到達できるよう支援されるアプローチ」と定義されています（Elwyn G et al. 2010）。

SDMには9ステップモデルやGoal-Based SDMなど複数の枠組みがありますが、HoffmannらはSDMに共通する主な要素を

①患者を安心させる
②問題点と予後の説明
③患者の期待の抽出
④検査や治療の選択肢の提示
⑤選択肢のメリット・デメリットの説明
⑥患者の価値観・状況に照らした検討
⑦患者への配慮

…の7つに整理しています（Hoffmann TC et al. 2020）。

このプロセスを経て合意形成が行われる点がSDMであり、単なる「患者さんと良好な関係性を築けたこと」とは異なる概念です。

しかし現状、臨床では「患者教育」や「医療者と患者の良好なコミュニケーション」をSDMと同義と捉える誤解が根強いとされ、研究でもそのズレが示されています。

国際的に観察されたSDM実践スコア（OPTION-5スコア）は、

オランダ：27/100（Hacquebord S et al. 2025）
イギリス：24/100（Jones LE et al. 2014）
ベルギー：5.2/100（Dierckx K et al. 2013）

…など非常に低い水準が報告されており、いかにSDMの実践が難しいかが示唆されています。

一方で、医療者自身は「自分はSDMをできている」と認識してしまいがちです。

2021年のオーストラリア研究では、372名の理学療法士の60%が「SDMをできている」と自己評価した一方で、SDMの中核である「選択肢のメリット・デメリットをエビデンスに基づいて説明する」を実施していた人は少数でした（Hoffmann T et al. 2021）。

つまり、「できているつもりだが、実際にはできていない」という状況が起きているということです。

これがなぜ起こるのかについて、先行研究では、「患者教育＝SDM」「良好なコミュニケーション＝SDM」といった誤解がSDMの普及を阻害しているため、と考察されています。

SDMとは、単に「患者に寄り添う」ことでも、「丁寧に説明すること」でもありません。

寄り添いや説明は医療職として当然の前提であり、SDMの本質は、エビデンス、治療選択肢、患者の価値観や状況を統合し、意思決定の過程そのものを共有することにあります。

そして、このプロセスをしっかりと学習し、実践できるセラピストが増えることが、倭国のリハビリにとって重要なテーマだと思います。
